While we cannot guarantee a favorable outcome, we will contact the federal agency on your behalf and see that you receive a fair and timely response to your inquiry.

For security and efficiency reasons, our office does not accept walk-in casework appointments. To get started on a case:
1) Fill out the digital privacy release form. If you don’t reside in AZ03, the application will not be processed.
2) Have a complicated case? Please call our office at (602) 956-2285 to provide a general recap and schedule your in-person appointment.

Please Note:
Before our office can contact a federal agency on your behalf, the Privacy Act of 1974 (5 U.S.C. § 552a) requires that Members of Congress or their staff have written authorization before they can obtain information about an individual's case. We must have a completed and signed privacy release form from you to proceed with this type of request. You can find the link to the privacy release form here.
After you complete and submit the privacy release form, it is directly uploaded into our system. Alternatively, you can print, fill out and mail the form to our office.
Once we receive the completed form and any supporting documentation you may want to include, one of our caseworkers will review your request and promptly submit the inquiry to the agency.
Please include all pertinent information and claim numbers in your correspondence—such as:
· Your full name, social security number and date of birth;
· Your address, phone number and daytime phone number (if different than home) so that we can obtain any additional information from you that might be necessary;
· Copies of any supporting documents or correspondence that you may have from the agency involved;
· For USCIS inquiries or requests for assistance involving a case number, include the reference number we can point to when making the inquiry;
· For request for assistance obtaining a passport, please include the passport locator number if you have it.
